We provide a team-based approach to care. This Team includes physicians, nurses, hospice aides, social workers, chaplains, bereavement counselors and trained volunteers. Our Hospice Care Team, which meet every two weeks to review care plans, are coordinated by a patient care manager. They work together to ensure that the patient’s physical, emotional and spiritual needs are met.

Hospice Physician
Works in conjunction with the patient’s primary physician and care team.

Registered Nurse
Manages the entire team caring for the patient and ensures satisfactory medical care. The Registered Nurse is highly skilled in assessing and managing pain/discomfort.

Certified Home Health Aide
Assists with personal care such as bathing, feeding, etc. Oversees dental and mouth care.

Social Worker
Assists in providing emotional care to patient & family members, helps address financial issues and governmental support (if qualified), and assists with end-of-life preparations such as funeral and mortuary arrangements.

Spiritual Counselor/Chaplain
Works in conjunction with the patient’s clergy and is available to provide spiritual support for patient and family.

Hospice Volunteer
Specially trained volunteers are available to spend time with the patient, offer companionship and provide relief for the family.

Dietician, Physical Therapist and Occupational Therapist
Available for consultation when necessary.
