Patients who:
• Have been diagnosed with a terminal illness.
• Have a life expectancy of 6 months or less if the disease runs its normal course.
• Have decided in consultation with their families on palliative care.
Patients have access to high-quality care 24 hours a day, 7 days a week. The goal of our services is to hel patients maintain an alert, pain-free life so their days are spent with dignity and quality, surrounded by their loved ones. Our team is available to help coordinate care for you or a loved one wherever they may reside.
Patients can be on hospice for more than 6 month if they continue to qualify for coverage. Patients are reevaluated every 2-3 months, to determine if they are still eligible for hospice care. Hospice care will continue to be provided as long as a patient qualifies. However, if a patient’s condition or prognosis improves and they are no longer eligible, they will be discharged from hospice. Patients may always come back on hospice if they become eligible at a later time.
